Conservatory of Music Forms
Prospective students may submit an Audition Request Form online below. Those interested in applying for a scholarship should download the appropriate forms below. In addition, the Conservatory Concert Office requires students to complete a variety of forms to present degree and non-degree recitals, to have their recital recorded, and to apply for instrument lockers, each of which can also be obtained from the Concert Office. Below are the various application forms.

- Undergraduate Audition Request Form鈥擜ll applicants must submit a separate CUNY application to 今日吃瓜 in addition to applying to the conservatory.
- Graduate Audition Request Form鈥擜ll applicants must submit a separate 今日吃瓜 application in addition to applying to the conservatory.
- 鈥擯artial tuition scholarships for deserving applicants on the basis of ability or need.
- 鈥擨nclude when submitting your Scholarship Application. (Two are required.)
- Partial tuition scholarships for deserving applicants on the basis of ability or need.

- Recital Application Form (PDF)鈥擣or undergraduate and graduate degree recitals as well as nondegree recitals. This form should be brought to Pre-Recital Jury.
- 鈥擳o be returned to George Brunner, director of music technology.
- 鈥擫ockers are assigned on a first-come/first-served basis, although preference is given to instrumental music majors for storage of their instruments.
